Practice Lead - Integrated Health and Social Services
Practice Lead- Integrated Health and Social Services - 32 hours/week over 5 days
The Practice Lead provides clinical and practice leadership across both Social Services and Primary Health services at Number 10 Southland Youth One Stop Shop.
The role supports safe, high quality, youth -friendly, and culturally responsive practice across all client -facing services. Working within a multidisciplinary environment, the Practice Lead strengthens collaboration between health and social services to ensure rangatahi receive integrated, holistic care aligned with Te Whare Tapa Wha and youth development best practice.
This role is ideal for an experienced practitioner who enjoys supporting other professionals, strengthening practice, and ensuring rangatahi receive safe, coordinated and holistic care
The Practice Lead provides:
• Practice leadership and clinical governance across services
• Case consultation and reflective practice support for staff
• Oversight of safeguarding, risk management, and complex cases
• Quality assurance and service improvement leadership
• A small caseload of complex rangatahi where specialist support or coordination is required
The Practice Lead works collaboratively with clinical staff and management to support safe practice but does not hold medical governance responsibly for health services.
Number 10 SYOSS has been providing holistic wrap-around support for rangatahi aged 10-24 for over 18yrs. We are a supportive, fun, friendly team who are making a difference in the lives of rangatahi in Murihiku.
Our team includes Youth Workers, Counsellor, Nurses, Doctor and a Project Coordinator.
